The scientific work that will be presented in Madrid, entitled “EffectoCy and Safety of An Hub and Spoke Circuit for NScemi Patient in Italy”, has aroused particular interest in the international scientific community for having demonstrated excellent results in a large number of patients suffering from N-STEMI type infarction, treated at the hemodynamics of Patti, inserted in a clinical path including the Hub Center (reference center) and all the other spoke centers in the area (relating centers); This is the largest comparative case studied ever published, with a total enrollment of 1702 patients divided into more or less equal parts between the study group and the control group. The route in question was implemented by Patti’s emodynamics in 2019 to face, as far as possible, the chronic deficiency of beds that exist in the Patti hospital. In fact, the “Baron Romeo” garrison, classified as the Hub Center for the presence of the core and reference of about 280,000 inhabitants, has never been able to absorb all the requests for hospitalization of floured patients, to be undergone urgent coronary a criminal, from Spoke centers, such as the hospital principals of Milazzo, Barcelona Pg, S. Agata di Militello and Mistretta. The clinical path, the subject of the publication, currently only partially in force for the unavailability of the Cardiology of S. Agata di Militello for almost a year, provides that the patient suffering from N-STEMI heart attack, coming from the spoke centers with indication to coronary artery, reaches the hemodynamics of Pacts by ambulance, performs the invasive procedure to return at the end of the exam at the hospital of origin (circuit (circuit “Spoke-hub-supke”), avoiding, on the one hand, the exclusive dependence on the beds of the Hub Center, on the other, providing the same greater availability of hospitalization for non-deferrable emergencies (e.g. STEMI type heart attacks). In about six years of observation in the study group belonging to the “Spoke-hub-Spoke” circuit, compared to the control group, composed of patients hospitalized in the same Hub center of Patti, no significant differences have been recorded in terms of the efficacy of treatment and safety (no increase in complications); Furthermore, thanks to the reduction of patients’ hospitalization days waiting for coronarography, considerable savings of economic resources and improvement of the prognosis of the disease were also achieved. In the scientific publication, the profitable intraospedal collaboration with the resuscitation of pacts and inter-hospital with the cardiologies of Milazzo, S. Agata di Militello and Mistretta of the Asp Messina also stands out, and with the cardiac surgery of the Papardo di Messina hospital, without which nothing would have been achievable: the collaboration at the service of the community for the improvement of clinical results. In the study, moreover, thanks to the strategic directions of the previous and current ASP Messina are reported, which responsibly shared, authorized and supported the clinical path in question. Considering the constant increase in the assistance offer, also by virtue of the imminent activation of a second room of hemodynamics, and the request for care, it would be desirable that in the reprogramming of the regional hospital network, a proportional and effective adaptation of the number of beds intended for heart patients of the vast Tyrrhenian area of the nearby Patti of Patti is caught.
